1.) RELEASE – I hereby recognize and acknowledge that my child’s participation in recreational activities may involve bodily injury and / or emotional injury to myself and / or my child. In consideration of my child being permitted to participate in such events, I for myself, my child, my heirs, my executors and administrators, hereby voluntarily and knowingly release, waive, and discharge Salt Lake Lutheran High School, Lynx Basketball Club, and its officers, employees, and volunteers from any and all liability that may result from my child’s participation in basketball club activities. In addition, I agree that my insurance company or I will pay for medical, hospitalization or any other expenses resulting from my child’s participation.
2.) Emergency treatment – I hereby authorize Salt Lake Lutheran High School or Lynx Basketball Club staff to act on my behalf in accordance with their best judgment in case of an emergency involving my child, and agree to assume full responsibility for all expenses, medical or otherwise, that may arise there from. I understand that my or my insurance will be billed for such emergency treatment.
3.) I hereby expressly agree that this release, waiver and indemnity agreement, is intended to be as broad and as inclusive as permitted by the laws of the state of Utah, and that by signing this assumption of risk and liability release, I acknowledge that I have read its contents and disclosure, and that I agree to its terms.
